At Modern Hero Home Care Solutions, we believe that home is more than just a place. It is where independence is cherished, dignity is preserved, and a lifetime of memories lives on.

Built on more than 20 years of experience in the Medicare health plan industry, Modern Hero Home Care Solutions brings together a deep understanding of the senior population, a commitment to quality, and a genuine passion for helping individuals age with dignity and independence.

Our care team

Every role is structured to make sure care is delivered safely, supervised closely, and coordinated smoothly. This reflects how our team is organized — not a personal directory.

Caregivers / Home Care Aides

HCA-registered caregivers who provide hands-on, day-to-day non-medical care according to each client's plan.

Lead Caregivers

Experienced caregivers who provide on-the-ground mentorship and support quality in the field.

Care Coordinators

Coordinate scheduling, match caregivers to clients, and serve as the contact point for families and partners.

Supervisors

Provide ongoing oversight through field visits, coaching, and care plan reviews.

Administrator / Alternate Administrator

Oversee agency operations and ensure compliance with California licensing requirements.

Accountability in Action

Quality, supervision & accountability

Accountability isn't an afterthought — it's built into how we train, supervise, and continuously improve the care we deliver.

Supervision of caregivers

Our caregivers are supported through scheduled and unscheduled field visits, documentation review of progress notes and logs, phone check-ins, performance evaluations, and ongoing coaching. Supervisors make sure every caregiver follows the written care plan and the safety protocols that protect our clients.

Quality assurance program

Modern Hero maintains a comprehensive QA program that includes monthly Quality Assurance reports, client satisfaction surveys, caregiver performance evaluations, incident trend analysis, care plan audits, and training compliance reviews. Findings feed directly into improvements in training, policies, and service delivery.

Complaint resolution

Clients and families may file a complaint verbally or in writing at any time. We acknowledge every complaint within 24 hours, investigate within 72 hours, provide a resolution, and follow up with the client. Retaliation is never permitted, and every complaint is reviewed as part of our Quality Assurance program.
