Our Process

How it works

A clear, careful process — from the first referral to a matched caregiver — so families always know what happens next.

Watch: How care gets started

  1. 1

    Referral received

Our office receives the referral and gathers basic client information to get started.

  2. 2

    Care planning appointment scheduled

We schedule a care planning appointment at a time that works for the client and family.

  3. 3

    Admission packet completed

The admission packet is completed so all required documentation is in place.

  4. 4

    Client assessment conducted

A thorough assessment covers physical status, cognitive status, ADLs/IADLs, home safety, and personal preferences.

  5. 5

    Personalized care plan developed

We develop an individualized care plan tailored to the client's specific needs and routines.

  6. 6

    Caregiver assigned

A caregiver is carefully assigned based on skills, experience, and the client's needs.
